Job summary

CareSource is hiring an Integrated Care Clinical Manager in Massachusetts to provide home-based, complex care management for dually eligible enrollees with medical, behavioral, and social needs. You will lead an interdisciplinary team, coordinate with physicians and community resources, and focus on seamless transitions and quality outcomes.

The role requires MA RN licensure, an AS in Nursing, and experience in care coordination; travel is expected to areas within the state and adjacent

Qualifications

AS in Nursing from an accredited program required
RN license in MA required
Case management experience preferred

Responsibilities

Engage with enrollees in their homes to establish complex care management relationships
Lead interdisciplinary care team to create holistic care plans
Coordinate with providers and community resources to ensure seamless care transitions
Monitor enrollee utilization of long‑term services and supports
Educate enrollees and families about benefits, chronic conditions, and preventive care
Travel to member, provider, and community visits as required
