Job summary

ARC in Middletown, CT seeks a Care Coordinator Manager to lead clinical care coordination across Renew Health + Recovery’s New England agencies. Supervisor roles include leading leads, assigning tasks, and reviewing assessments while facilitating weekly Care Coordinator meetings.

Requirements include a Master’s degree (preferred), 2 years of home/community health care in the past 5 years and 1 year psychiatric experience, and licensure (RN/LCSW or equivalent).

Qualifications

• Master's degree preferred.
• Minimum of 2 years of home or community-based health care experience within the past 5 years and a minimum of 1-year psychiatric experience.
• Substance Use Disorder treatment knowledge preferred.
• RN, LCSW or state-specific equivalent with an active license to practice in the state.

Responsibilities

• Supervision of Clinical Care Coordinator Leads/Clinical Care Coordinators across Renew's New England agencies.
• Assign and track Clinical Care Coordinator assignments.
• Review weekend assessments with Integrated Healthcare Services Director weekly.
• Facilitate weekly Care Coordinator meetings.
• Identify training needs and discuss with Training Team.
