Job summary

Humana Inc. is seeking a Telephonic Care Coach to join CenterWell Home Health on a remote team. You will support members’ wellness by coordinating services and resources, and work with caregivers and providers to remove barriers to care.

Responsibilities include monitoring progress through assessments and conversations, following established procedures, and applying professional standards to stay compliant while delivering compassionate care from a home-based setting.

Qualifications

• 2+ years of related experience.
• Proficiency with Microsoft Office programs (Outlook, Word, Excel, SharePoint and Teams).
• Experience with electronic information applications/software.

Responsibilities

• Support a member's optimal wellness state by coordinating services & resources.
• Identify and resolve barriers that hinder effective care.
• Ensure patient is progressing toward outcomes by monitoring through assessments and conversations.
• Understand work area concepts, standards, regulations and operating procedures.
• Make decisions regarding own work priorities and follow directions.
• Work guided by precedent, procedures, regulations and professional standards with some interpretation.
