Humana is seeking a Care Coach for Humana Gold Plus-Integrated in Illinois to support members’ health and wellness by conducting comprehensive assessments, identifying needs, and guiding members and families to resources for optimal care. You will report to the Manager, Care Management and join a dedicated team focused on compassionate, patient-centered service.

Responsibilities include telephonic care management, removing barriers to care, and monitoring progress toward outcomes.

Competenze

• Bachelor's degree in a health-related field is required.
• 2+ years of case management or healthcare support experience.
• Proficiency with MS Office (Word, Excel, Outlook).

Mansioni

• Conduct telephonic care management and assessments to identify member needs.
• Identify and resolve barriers to effective care.
• Monitor progress toward outcomes with ongoing care planning and data reviews.
• Adhere to guidelines, procedures, and regulatory standards.
