Job summary

Humana is seeking a Care Coach to evaluate members' needs and guide them toward resources to support wellness. This is a remote role with four weeks of training from 8:30 AM to 5:00 PM EST, followed by hours 10:00 AM–6:30 PM EST, Monday–Friday.

You will perform outreach, assess needs, develop care plans, coordinate services, and monitor progress while ensuring compliance in a regulated program.

Qualifications

• Associate degree in any field.
• Minimum 2 years of direct adult patient care experience.
• Experience interacting with members of the healthcare team.
• Strong computer proficiency and ability to quickly learn EMR systems.
• Exceptional communication and interpersonal skills.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess member needs and develop individualized care plans.
• Coordinate services and connect members to clinical and community resources.
• Identify and resolve barriers that impede care and independence.
• Monitor progress toward health outcomes and adjust care strategies.
• Maintain compliance within a regulated program.
• Navigate multiple computer systems while engaging members in a structured environment.
