Job summary

Humana is seeking a Care Coach to evaluate members’ needs and guide them toward resources that support optimal wellness. This remote role focuses on coordinating with clinical and community services to maintain compliant care plans for members with chronic conditions.

You will assess needs, develop individualized plans, monitor progress, and navigate multiple systems while communicating effectively with members and the health care team. Training runs 8:30 AM–5:00 PM EST for the first month.

Qualifications

• Associate degree required.
• Minimum 2 years of direct adult patient care experience.
• Experience interacting with members of the healthcare team.
• Strong computer proficiency and ability to learn and navigate electronic software including EMR.
• Exceptional communication and interpersonal skills.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions and barriers.
• Assess member needs and develop individualized care plans.
• Coordinate services with clinical, community, and multidisciplinary resources while ensuring compliance.
• Identify and resolve barriers to care and independence.
• Monitor progress and adjust care strategies as needed.
• Maintain adherence to compliance within a regulated program.
• Navigate multiple computer systems while engaging members in a fast-paced environment.
