Job summary

Humana is seeking a Care Coach to assess member needs, develop care plans, and coordinate services with clinical and community resources. You will operate remotely, ensuring compliance in a highly regulated program and delivering member-centered care with strong communication and digital skills.

The role supports a 40-hour workweek on weekdays (EST). Initial training runs 8:30 AM–5:00 PM EST, with potential occasional travel for training or meetings.

Qualifications

• Associate degree required.
• 2+ years direct adult patient care experience.
• Strong computer proficiency with EMR systems.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions.
• Assess needs and develop individualized care plans.
• Coordinate services and connect to clinical/community resources.
• Identify and resolve barriers to care and independence.
• Monitor progress and adjust care strategies.
• Maintain strict compliance with regulated program requirements.
• Navigate multiple computer systems while engaging members.
