Job summary

Humana Inc. is seeking a Care Coach 2 to assess and guide members toward services that support optimal wellness. The role requires establishing member-specific care plans and coordinating with providers, payers, and interdisciplinary teams.

You will visit Medicaid members in homes, assisted living, and long-term care facilities, with substantial local travel. Advanced clinical licensure (LCSW/LMFT/LCPC) and 2+ years of behavioral health case management are required, along with proficiency in EMR

Qualifications

Active Illinois license in LCSW, LMFT or LCPC (No supervisees or provisional licenses)
2+ years of post-degree clinical experience in behavioral health setting
Case management experience with complex SMI, SUD, SED populations
Ability to travel regionally to facilities and homes for assessments
Proficient with Microsoft Word, Outlook, and Excel
Valid driver’s license and reliable transportation

Responsibilities

Assess and evaluate member needs to establish a member-specific care plan.
Coordinate services and monitor the case management plan against goals.
Visit Medicaid members in homes, assisted living facilities, and long-term care facilities (75–90% local travel).
Guide members and families toward appropriate resources and minimize restrictive settings.
Use data, conversations, and assessment to ensure progress toward desired outcomes in care planning.
