Job summary

Humana Inc. in Wisconsin seeks a Manager, Care Coaching to lead a team of Field Nurses and Care Coaches within the Family Care program. You will mentor staff, ensure DHS contract compliance, and drive quality of care management.

The role offers remote-from-home work with occasional travel to Humana offices for trainings, and a typical schedule of Monday–Friday, 8:00 am–4:30 pm CST. Manage ~11–15 associates and collaborate across departments.

Qualifications

• Bachelor's degree in human services or related field required
• 1 year of leadership experience in human services required
• 3 years of experience in the Family Care program required

Responsibilities

• Mentor and support Field Nurses and Care Coaches to ensure DHS contract compliance
• Identify learning needs and arrange mentorship; facilitate orientation and ongoing learning
• Assess staff competencies and performance; review reports and improve care quality
• Participate in cross-department collaboration; educate community partners
• Assist in hiring, orientation and training of associates
• Facilitate unit meetings and attend quality visits
