This full-time role supports statewide coverage from a remote location, serving members, providers, and staff with professional, culturally appropriate communication.

The position emphasizes customer service, documentation of activities, and timely referrals, with responsibilities including scheduling assistance and resource guidance

Qualifications

• Requires high school diploma or GED.
• Prior telephonic call center experience preferred.
• Healthcare experience and medical terminology knowledge are highly preferred.
• Managed care experience is a plus.

Responsibilities

• Conduct continual telephonic outreach to identified members for care management.
• Receive and manage referrals from callers and data reports.
• Document screenings and refer needs to clinical care management staff.
• Verify demographic information and send educational materials to members.
• Assist with scheduling medical appointments and transportation as needed.
• Provide information on community resources and navigate care coordination.
