Qualifications

• High School Diploma or GED required.
• Prior telephonic call center experience preferred.
• Healthcare setting experience recommended.
• Bilingual ability preferred.
• Knowledge of care management a plus.

Responsibilities

• Conduct continual telephonic outreach to identified members for engagement with care management services.
• Receive and manage referrals, linking members with care management needs.
• Document screenings and refer clinical needs to the appropriate team member.
• Verify demographic information and maintain accurate records.
• Assist with scheduling medical appointments and transportation where needed.
• Ensure HIPAA compliance in all communications.
• Meet monthly productivity and role expectations.
