Community Care of North Carolina is seeking a Care Manager 1 - Non-Clinical to provide statewide care management for Medicaid enrolled members. The role requires collaboration with providers, families, and community teams to coordinate seamless health care across services.

The position is field-based with remote work when not providing services; occasional in-person trainings and travel within North Carolina are expected. Bachelor's degree in a health-related field or RN licensure is required.

Qualifications

• Bachelor's degree in health, psychology, sociology, social work, nursing, or related field.
• RN licensure is acceptable; CCM certification preferred.
• Must reside in NC or within 40 miles of the NC border.
• Experience with case management and multi-disciplinary teams is preferred.

Responsabilités

• Provide statewide care management across physical, behavioral health, and social services.
• Complete member assessments and plan the continuum of care.
• Develop, review, implement, and monitor member care plans with care teams.
• Coordinate referrals to community-based services and resources.
• Maintain documentation in the Care Management platform and meet productivity expectations.
