Job summary

Community Care of North Carolina (CCNC) is seeking a Care Manager 1 - Non-Clinical to provide statewide care management for Medicaid enrolled members in North Carolina. The role is field-based with remote work when not in direct service, requiring travel in-state and occasional in-person trainings.

The Care Manager will collaborate with primary care providers, guardians, and families to coordinate integrated health care.

Qualifications

• Bachelor's degree in health-related field or licensure as an RN.
• 2 years of experience with child welfare population preferred.
• Must reside in NC or within 40 miles of the NC border.

Responsibilities

• Provide integrated whole-person care management across physical and behavioral health.
• Complete member assessments considering medical, biopsychosocial, behavioral, and cultural needs.
• Develop and monitor care plans in collaboration with members, caregivers, and multidisciplinary teams.
• Provide education to members and families about diagnoses, medications, resources, and self-management.
• Maintain documentation in the care management system in compliance with policies.
• Travel within NC as required for field responsibilities.
