Job summary

Community Care of North Carolina is seeking a Supervising Care Navigation professional to oversee remote operations across the state. You will manage performance metrics, workflows, and escalation paths to ensure high engagement and outcomes for rising risk members.

Work with care management, practice, quality improvement, and provider relations teams to coordinate referrals, access EMR systems, and safeguard patient data while guiding staff with coaching and development.

Qualifications

• RN with NC license; BSN preferred.
• 2+ years in case/care management or related healthcare experience.
• CCM certification preferred; to be obtained within 2 years of hire.
• Experience with EMR/HIE systems and data access.

Responsibilities

• Monitor program measures and focus on rising risk populations.
• Develop and implement new workflows and programs to support success.
• Supervise care navigation staff and conduct performance evaluations.
• Coordinate with care management, practice, quality improvement and provider relations teams.
• Ensure access to Hospital/EMR/HIE systems and safeguard patient data.
• Provide coaching and staff development; maintain productive team dynamics.
