Acentra Health, LLC is seeking a Community Engagement Coordinator to connect California's local healthcare delivery system with community organizations and providers serving the Fee-for-service population.

The role collaborates with care teams to support high-risk member transitions, advocate for beneficiaries, and promote timely access to care. Remote role for CA residents, with preference for Northern California, and bilingual Spanish skills are welcomed.

Qualifications

• Patient Navigator, CCM or Social Worker experience.
• 2+ years community healthcare engagement experience.
• Field-based focus on building community partnerships.
• Must reside in California.
• Strong written and verbal communication skills.
• Customer service oriented.
• Willing to travel within California.

Responsibilities

• Field-based engagement specialist to develop community organization relationships.
• Develop strategies meeting members where they live and build trusted engagement.
• Collaborate with physicians, care coordinators, community health workers and social workers.
• Educate members about community resources and advocate for beneficiaries.
• Coordinate transitions of care and ensure timely access to services.
• Maintain confidentiality and comply with HIPAA and policies.
• Document status updates and other documentation per regulations.
