Acentra Health is seeking a Community Engagement Coordinator in California to build relationships with local healthcare providers and community organizations. You will support transitions of care, coordinate with care teams, and advocate for beneficiaries in a remote-friendly setup with CA residency preferred for on-site interactions.

Ideal candidates have patient navigation experience, strong communication, and a willingness to travel locally within California.

• Experience as a Patient Navigator, CCM, or Social Worker.
• 2+ years in community healthcare engagement.
• Must reside in California.
• Strong written and verbal communication skills.
