The position is remote with residency in California preferred, especially Northern California areas like San Francisco, Santa Cruz, Sacramento, and the Central Valley.

Qualifications

• Required qualifications include Patient Navigator, CCM or Social Worker experience.
• 2+ years of community healthcare engagement experience.
• Field-based healthcare experience focused on developing partnerships.
• Must reside in California and travel locally as needed.
• Strong written and verbal communication and customer service skills.

Responsibilities

• Develop and enhance community-based organization relationships.
• Create community engagement strategies aligning with members' lives.
• Collaborate with physicians, care coordinators, and social workers to meet beneficiary needs.
• Educate members about community resources and advocate for them.
• Coordinate transitions of care and ensure timely access to services.
• Maintain confidentiality and comply with HIPAA and policies.
• Document status updates and notifications per regulatory requirements.
