Acentra Health is seeking a Community Outreach, Supervisor to oversee statewide outreach initiatives supporting a culturally diverse Medi-Cal population. This role provides leadership over outreach specialists and coordinates with Provider Outreach, Care Coordinators and case management staff to ensure compliance with state guidance and contract performance standards.

Qualificações

• Associates degree from an accredited college or university OR will consider 6 years of experience in lieu of a degree.
• A minimum of 4+ years of supervisory, lead, or team leadership experience in a healthcare, human services, care coordination or related environment.
• A minimum 2+ years of experience in public outreach, community engagement, or external affairs.
• Demonstrated experience overseeing consumer-facing programs and staff.
• Strong knowledge of Medicaid, healthcare delivery systems, or public sector programs.
• Experience working with diverse populations, including non-english speaking populations.
• Ability to work independently and effectively lead a fully remote team.
• Flexibility to travel up to 25% within CA as needed.

Responsabilidades

• Maintain outreach team goals and performance standards and provide regular feedback on performance SLAs.
• Understand unique needs of Medi Cal members experiencing transition of care needs and work with stakeholders, leadership, Managed Care Organizations (MCOs), and Community-based organizations (CBOs), and advocacy groups.
• Maintain stakeholder - Coordinator relationship map and ensure the ongoing capture of partnership outreach.
• Advise leadership on environmental challenges and opportunities as well as creation of engagement opportunities.
• Support engagement strategies that improve appointment adherence and completion rates.
• Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.
