Job summary

Acentra Health seeks a Community Engagement Coordinator in California to build relationships with local healthcare delivery systems and community organizations, focusing on integration with providers serving the Fee-for-service population.

You will engage with care teams to support high-risk member transitions, education on community resources, and coordination of timely access to care while protecting member confidentiality.

Qualifications

• Patient Navigator, CCM or Social Worker experience.
• 2+ years community healthcare engagement experience.
• Field-based healthcare experience focused on developing and strengthening community partnerships.
• Must reside in California.
• Willing to travel locally within California as needed.

Responsibilities

• Develop and strengthen community-based organization relationships.
• Create engagement strategies meeting members where they live and establish trusted environments for healthcare engagement.
• Collaborate with physicians, care coordinators, CHWs, case managers, pharmacists, and social workers/educators to align beneficiary needs with health services.
• Educate members about community resources and advocate on their behalf.
• Coordinate transitions of care to ensure timely access to care and maintain open communication with all parties.
• Maintain confidentiality and comply with HIPAA and company policies.
