Job summary

TAG MedStaffing is seeking a bilingual Social Care Coordinator to support member engagement, care coordination, and access to healthcare services. This remote role requires residence within commuting distance of New York City and fluency in English and Spanish.

You will handle member intake, coordinate referrals, track service delivery, and ensure HIPAA-compliant documentation while working Monday to Friday with occasional Saturdays.

Qualifications

• 3+ years of customer service or care coordination experience.
• Knowledge of Medicaid programs, healthcare services, and community resources.
• Proficiency with CRM, care management, or contact center software.
• Fluency in English and Spanish (speaking, reading, and writing).
• Strong communication, organizational, problem-solving and active listening skills.
• Ability to manage multiple priorities in a remote environment.

Responsibilities

• Complete member intake and support enrollment in healthcare and social service programs.
• Schedule assessments, coordinate follow-up activities, and assist members with accessing services.
• Coordinate referrals with healthcare teams, community organizations and external providers.
• Manage inbound and outbound communications, respond to inquiries, and provide ongoing program assistance.
• Monitor referral progress, confirm service delivery, and document interactions and outcomes.
• Maintain confidential member records and ensure compliance with Medicaid, HIPAA and related documentation requirements.
