Job Overview – Social Care Coordinator (Bilingual) Compensation: $52,000/year Location: Remote Schedule: Monday to Friday (Remote), Occasional Saturdays with a Weekday Off

TAG MedStaffing is hiring a Social Care Coordinator for our client, supporting member engagement, care coordination, and access to healthcare and community-based services. This remote role will assist members with service navigation, non-clinical assessments, referrals, and ongoing program participation while maintaining accurate documentation. Candidates must reside within commuting distance of New York City and be fluent in both English and Spanish.

Responsibilities as the Social Care Coordinator:

• Member Intake: Complete member intake, maintain accurate records, and support enrollment in healthcare and social service programs.
• Care Coordination: Schedule assessments, coordinate follow-up activities, and assist members with accessing available services.
• Referral Management: Coordinate referrals with healthcare teams, community organizations, and external service providers.
• Member Support: Manage inbound and outbound communications, respond to member inquiries, and provide ongoing program assistance.
• Service Tracking: Monitor referral progress, confirm service delivery, and maintain accurate documentation of member interactions and outcomes.
• Regulatory Compliance: Maintain confidential member records and ensure compliance with Medicaid, HIPAA, and applicable healthcare documentation requirements.

Qualifications for the Social Care Coordinator:

• Experience: 3+ years of customer service, member engagement, patient support, or related experience is required, with healthcare or care coordination experience preferred.
• Industry Knowledge: Knowledge of Medicaid programs, healthcare services, and community-based resources is required.
• Technical Skills: Proficiency in CRM, care management, or contact center software is required.
• Language Skills: Fluency in English and Spanish, including speaking, reading, and writing, is required.
