TAG MedStaffing is hiring a Social Care Coordinator (Bilingual) in New York, NY for our client. This role supports individuals with care coordination, community resource referrals, service navigation, and ongoing engagement.

The Social Care Coordinator will connect individuals with resources, maintain documentation, and provide timely follow-up to enable access to services. The position requires English and Spanish fluency and 3+ years in customer service, member engagement, or care

Competenze

• High school diploma or equivalent; associate or bachelor's in Human Services preferred.
• Fluency in English and Spanish (reading and writing) required.
• 3+ years of customer service, member engagement, or care coordination experience.
• Knowledge of Medicaid, healthcare navigation, or community resources required.
• Experience with Five9 and CRM or care management platforms.

Mansioni

• Schedule non-clinical assessments and provide ongoing support to members throughout program participation.
• Connect members with healthcare, nutrition, and community-based services based on needs.
• Coordinate referrals with internal teams and external providers while tracking completion and follow-up.
• Manage inbound and outbound communications to answer questions and provide program guidance.
• Maintain accurate member records, referral activity, service updates, and documentation in care management systems.
• Follow Medicaid, HIPAA, privacy, and healthcare documentation requirements.
