Legacy Community Health is seeking a Bilingual Health Navigator - PrEP to provide compassionate, virtual patient navigation, education, and support for HIV prevention services. You will guide patients through PrEP initiation, adherence, and retention while collaborating with Legacy Pharmacy and program staff to remove barriers and improve outcomes.

The role emphasizes empathetic communication, culturally competent guidance, and proactive follow-up to meet grant and program goals.

Qualifications

• High School Diploma or equivalent.
• 3–5 years of relevant experience.
• Bachelor's degree in related field preferred.

Responsabilités

• Conduct virtual outreach to identify, educate, and recruit individuals for PrEP and HIV prevention services.
• Provide remote patient navigation to PrEP care, including scheduling and coordination with clinical teams.
• Deliver health education on PrEP, HIV prevention, and safer sex practices.
• Support patient retention through follow-up and reminders.
• Document all patient interactions in the EHR accurately.
• Collaborate with care teams to ensure a seamless patient experience.
