Job summary

GMHC is seeking a Client Navigator to connect Testing Center clients with PrEP/PEP, HIV care, and related social services. The role focuses on telehealth engagement, retention in care, and navigation support across on-site and outreach settings.

The candidate should be bilingual in English and Spanish, with knowledge of HIV/STIs and harm reduction. This position requires strong data entry, confidentiality, and teamwork to improve outcomes.

Qualifications

• High School Diploma required; PrEP/nPEP training certificate preferred.
• Bilingual English/Spanish with strong HIV/STI knowledge and the ability to provide education and referrals.

Responsibilities

• Facilitate linkage to care and navigation for PrEP, HIV/AIDS, or Hepatitis C clients, overcoming insurance barriers.
• Follow up telehealth clients who miss appointments and re-engage in care.
• Maintain accurate data in EHR systems and ensure confidentiality per HIPAA and NYS law.
• Provide HIV/STI counseling, testing, and referrals on-site and during outreach events.
• Attend CQI meetings and participate in trainings to improve navigation and harm reduction services.
