Job summary

Community Healthcare Network is seeking a PrEP Navigator to conduct outreach, assist with intakes, and educate clients seeking PEP/PrEP services. You will help coordinate medical appointments, troubleshoot insurance issues, and enter data into AIRS and the CHN EMR.

This role requires confidentiality, strong communication skills, and the ability to work with diverse communities. Requirements include a High School Diploma or GED, knowledge of HIV prevention, and proficiency with Word/Excel;

Qualifications

• Member of the targeted communities is preferred.
• Basic knowledge of HIV prevention including PEP/PrEP.
• Strong verbal and written communication skills.
• Familiarity with geographic area and community languages helpful.
• Proficient in Word and Excel; data entry in AIRS required.
• Respect for patient confidentiality and autonomy.
• Ability to communicate respectfully with diverse populations.
• Bilingual abilities are a plus.

Responsibilities

• Conduct outreach and inreach activities for PEP/PrEP services.
• Assist with intakes, enrollment, service navigation and follow-up.
• Enter and maintain data in AIRS and CHN EMR systems.
• Coordinate medical services with clinic staff.
• Participate in health events and community outreach activities.
• Maintain confidentiality of all patient information.
• Support retention and adherence counseling for PEP/PrEP patients.
• Comply with Employee Health Services and other duties as assigned.
