Zusammenfassung

Equitas Health is seeking a Prevention Health Navigator to lead health navigation services, outreach, screening, enrollment, and coordination of referrals for HIV-related programs. You will educate the community about PrEP, linkage to care, and enroll eligible participants into programs onsite and offsite.

The role requires coordinating with care providers, conducting client-centered HIV test counseling, and maintaining certification as a Federally Qualified Health Insurance Navigator.

Qualifikationen

• Associate degree with 1+ year relevant experience, or two years with coursework toward an associate degree, or 3+ years with high school diploma/GED.

Aufgaben

• Provide health navigation and support to individuals living with HIV to keep them engaged in care and achieve viral suppression.
• Offer referrals to HIV screening programs and care coordinators.
• Assist eligible clients with insurance options through Federal Marketplace and public programs.
• Document and track applications and submissions.
• Collaborate with internal teams and community partners for outreach and programming.
• Participate in biweekly supervision and monthly group supervision.
