Men’s Health Foundation seeks a PrEP Navigator to enroll clients in HIV biomedical prevention services at our West Hollywood clinic. Under supervision of the HIV Prevention Care Coordination Manager, you will assist with PrEP/PEP enrollment, eligibility, and linkage to care.

The role emphasizes outreach, documentation in EMR, and collaboration with prevention teams. A bachelor’s degree or equivalent outreach experience is required; willingness to work in a diverse, inclusive environment is

Qualifications

• Bachelor’s degree or three years outreach experience with vulnerable populations.
• Experience with people living with HIV/AIDS preferred.
• Ability to elicit information quickly and enter data accurately into EMR systems.
• Strong written and interpersonal communication skills.
• Proficiency in Microsoft Office suite.
• Commitment to the mission and values of Men’s Health Foundation.
• Ability to serve diverse populations in a multicultural environment.

Responsabilités

• Provide individual counseling/assessments for potential PrEP/PEP clients.
• Assess benefits eligibility and provide PrEP coverage navigation including enrollment and referrals.
• Assist patients with access to supportive services including mental health, housing, and nutrition.
• Document services in Allscripts and county/state databases as required.
• Collaborate with team to identify client needs and engage new patients.
• Conduct PrEP-related presentations and staff/community training.
• Attend conferences/trainings to stay updated on biomedical prevention trends.
• Coordinate community outreach activities to raise PrEP/PEP awareness and linkage to care.
• Represent MHF in community planning groups and boards.
• Co-produce outreach and recruitment events for PrEP/PEP programs.
• Participate in team and organizational meetings.
• Assist Benefits Department with insurance enrollment and provider changes.
• Other duties as required.
