Job summary

Fenway Health's HIV Prevention program seeks an HIV Prevention Navigator to support PrEP access and engagement within the Infectious Disease team. The role focuses on linkage to PrEP services, adherence support, insurance navigation, and coordinating care across providers, pharmacy, and community resources.

The Navigator educates patients on oral and long-acting injectable PrEP, manages referrals, and helps ensure equitable access to prevention services for diverse populations.

Qualifications

• Experience in HIV prevention or care navigation is desirable.
• Proficiency in coordinating with multidisciplinary teams.
• Strong documentation and data entry skills.

Responsibilities

• Engage patients and educate about PrEP options, adherence, and prevention strategies.
• Coordinate PrEP navigation from referral through initiation and retention in care.
• Assist with insurance benefits, prior authorizations, and patient assistance programs.
• Support long-acting injectable PrEP navigation and scheduling.
• Maintain accurate documentation in the electronic health record (EHR).
• Promote health equity and referrals to community resources.
