Humana Inc. is seeking a Bi-lingual Care Management Support Assistant to work remotely with English- and Spanish-speaking members. You will handle calls, complete non-clinical assessments, identify barriers, and help members access benefits and resources to improve health outcomes.

The role requires compassion, strong communication, and the ability to work in a call center with a shift from 11:30 AM to 8:00 PM EST; occasional travel to Humana offices may be required.

Qualifications

• Bilingual proficiency in English and Spanish and able to pass language assessments.
• Minimum of 1 year of experience managing high-volume inbound and outbound calls.
• Experience documenting and updating member information across multiple applications.
• Experience using Microsoft Word, Excel, Teams, and Outlook.
• Ability to work an 8-hour shift from 11:30 AM to 8:00 PM EST, including a 30-minute lunch.

Responsabilités

• Engage members by phone to encourage participation in care management programs.
• Complete required assessments and questionnaires to identify health risks and care needs.
• Assist members with scheduling doctor’s appointments or connecting with resources.
• Identify and address barriers to preventive screenings, benefits, and services.
• Support members in understanding and accessing benefits for overall well-being.
• Manage inbound and outbound calls and document member information.
