Humana Inc. seeks a Care Coach 1 to evaluate member needs, coordinate care, and guide families toward appropriate resources to sustain optimal wellness and patient safety. This is a remote, 40-hour workweek role with occasional travel for training.

You will monitor progress, manage care plans, and maintain HIPAA-compliant documentation across multiple systems. Requirements: health and human services degree or IL-licensed LPN, 2+ years of healthcare experience, bilingual English plus another

Qualifications

• Bilingual English with Spanish, Polish, Arabic, Creole, or other languages.
• Bachelor's degree in health and human services or LPN licensed in IL.
• 2+ years of healthcare experience.
• Knowledge of Microsoft Word and Excel.
• Exceptional verbal and written communication and interpersonal skills.
• Ability to use various electronic information applications.
