Qualifications

• Master's degree in behavioral health, social work or health care management.
• Independent clinical behavioral health license in WA (LMHC/LICSW/LMFT/Psychologist).
• Experience leading, building and implementing programs and delivering outcomes.
• Health coaching or medical social work experience preferred.
• Experience with suicide prevention and crisis in adolescent and young adult populations, as well as severe mental illness population preferred.
• Experience managing interdisciplinary care teams preferred.

Responsibilities

• Act as player/coach, building the team’s skills and directly supporting members in challenging situations.
• Create service plans using member information and community resources to increase relapse prevention.
• Educate members about health issues and available resources.
• Monitor and record member progress against plans and goals.
• Lead enrollment activities and ensure KPIs are met.
• Lead internal case reviews and coordinate with health plan partners as needed.
