Equity Staffing Group is seeking two Remote Behavioral Health Case Managers to support members with complex behavioral health needs. This 100% remote role centers on assessments, care planning, advocacy, and connecting individuals to services, with no direct treatment or in-person meetings.

Candidates may be experienced Behavioral Health RNs or independently licensed Social Workers. Licensure in the state of residence is required, with Wisconsin familiarity preferred for regional coordination.

Competenze

• At least 5 years of clinical nursing or social work experience with a focus on behavioral health.
• Experience working with vulnerable and complex populations.
• Licensure as RN or Social Worker as required by pathway.

Mansioni

• Conduct comprehensive behavioral health assessments to identify needs, risks, and resources.
• Develop and monitor individualized care plans based on member needs.
• Navigate members through the behavioral health system and connect with providers and resources.
• Coordinate services across the healthcare continuum with families and interdisciplinary teams.
• Maintain documentation of assessments, interventions, and care plan updates.
• Advocate for members and ensure appropriate healthcare resource utilization.
