Job summary

Ascend Healthcare is seeking a Remote Case Manager to coordinate comprehensive behavioral health services for patients nationwide. You will work with clients, families, and community partners to improve access to services, reduce barriers, and support engagement in treatment.

Responsibilities include documentation, assessments, and care coordination across medical, behavioral health, housing, and social support services.

Qualifications

• Bachelor's degree in social work, psychology, counseling, human services, public health, or 3+ years of behavioral health experience.
• Knowledge of community resources and social service systems.
• Strong organizational, communication, and documentation skills.
• Ability to establish rapport with diverse populations and maintain professional boundaries.

Responsibilities

• Maintain accurate, timely documentation compliant with policies.
• Conduct case management assessments to identify needs and goals.
• Coordinate medical, behavioral health, housing, and social services.
• Provide outreach to engage patients in treatment.
• Participate in staff meetings and quality improvement initiatives.
• Support crisis response within scope.
