Qualifikationen

• Bachelor’s degree in a human services field.
• Valid Texas Driver’s License.
• 2+ years of social service experience preferred.
• Crisis intervention and case management experience preferred.

Aufgaben

• Collaborate with the Community Behavioral Health Services Department and MDT to coordinate client care.
• Provide case management for individuals with housing instability and mental health/substance use needs.
• Provide appropriate referrals and maintain confidential case records.
• Consult with providers on resource-related issues and community integration; ensure risk management and crisis intervention.
• Deliver services via telehealth as required.
