Job summary

Author Health, LLC is seeking a qualified Care Manager (RN) to coordinate care for adults with behavioral health needs in a fully remote role. You will work with interdisciplinary teams to develop and monitor personalized care plans, aiming to improve health outcomes and reduce avoidable hospital utilization.

The ideal candidate has an active Florida or multi-state RN license, 3+ years in behavioral health or medical case management, and CCM or similar credentials.

Qualifications

• Active RN license in Florida or a multi-state compact.
• 3+ years in behavioral health or medical case management.
• CCM or similar credential.
• Proficiency with electronic health records.
• Bilingual in Spanish-English or Haitian Creole-English.
• Experience in hospital or outpatient settings.

Responsibilities

• Perform comprehensive assessments of behavioral health and medical needs.
• Remove barriers to engagement in care to ensure adherence.
• Create personalized care plans with patients, caregivers, and providers.
• Oversee and coordinate the execution of care plans to achieve goals.
• Perform comprehensive medication reconciliation by reviewing histories to ensure safety during transitions.
• Educate patients on health conditions, treatment options, and self-management.
• Advocate for timely access to medical and behavioral health services.
• Collaborate with psychiatrists, NPs, LCSWs, and field workers.
• Monitor progress and adjust care plans as needed.
• Participate in case conferences and quality improvement initiatives.
