Job summary

Blue Cross and Blue Shield of Minnesota is seeking a Case Manager - Behavioral Health to provide telephonic support to members with behavioral health needs. You will conduct assessments, identify gaps in care, and develop individualized care plans while coordinating referrals and connecting members to in-network providers.

This is a remote telework role with licensure options (RN/LMFT/LICSW/LP/LPCC) and requires 3+ years of case management experience alongside a bachelor's degree.

Qualifications

• Bachelor's degree required.
• RN/LMFT/LICSW/LP/LPCC licensure required or equivalent experience.

Responsibilities

• Provide telephonic engagement and support, using motivational interviewing to encourage participation.
• Develop and monitor individualized care plans with follow-up actions and goals.
• Navigate multiple systems to gather information, update case documentation and prepare for outreach calls.
