Equity Staffing Group is seeking two Remote Behavioral Health Case Managers to support members with complex behavioral health needs. This 100% remote role centers on assessments, care planning, advocacy, and connecting individuals to services, with no direct treatment or in-person meetings.

Candidates may be experienced Behavioral Health RNs or independently licensed Social Workers. Licensure in the state of residence is required, with Wisconsin familiarity preferred for regional coordination.

Remote Behavioral Health Case Manager!

Make a Meaningful Impact in Behavioral Health! Are you an experienced Behavioral Health RN or independently licensed Social Worker who is passionate about helping individuals navigate complex healthcare needs? We are seeking two Behavioral Health Case Managers to join a specialized team supporting members with significant behavioral health needs.

Pay: $45-$55/hour

Contract: Long-term contract approved through December 2027

Conversion Potential: Opportunity for consideration for permanent employment

Potential Permanent Salary: $75,000-$110,000 annually, depending on experience and licensure

Schedule: Monday-Friday, standard business hours (8:00 AM-5:00 PM CST)

Location: Fully Remote - Wisconsin candidates preferred, particularly those familiar with Dane County and the Madison area

This is a 100% remote, telephonic case management position. There are no in-person member meetings, and you will not be responsible for diagnosing or providing direct treatment. Instead, you'll focus on assessments, care planning, member advocacy, and connecting individuals with the services and resources they need.

• Conduct comprehensive behavioral health assessments to identify member needs, barriers, risks, and available resources.
• Develop, implement, and monitor individualized care plans based on each member's needs and established plan of care.
• Help members navigate the behavioral healthcare system and connect them with appropriate providers and community resources.
• Coordinate services across the healthcare continuum, collaborating with members, families, caregivers, providers, and interdisciplinary teams.
• Support members through complex care situations and transitions, helping ensure continuity of care.
• Identify unmet physical health or medical needs and coordinate with other case management teams as appropriate.
• Maintain ongoing communication with members and providers to monitor progress and address barriers to care.
• Complete thorough documentation of assessments, interventions, outreach, and care plan updates.
• Advocate for members while supporting appropriate healthcare resource utilization and positive outcomes.

Candidates must meet one of the following education and licensure pathways:

Registered Nurse (RN):

• Associate or bachelor's degree in nursing.
• Current, unrestricted RN license in the state of residence.

Social Worker:

• Master's degree in Social Work.
• Current, unrestricted advanced practice or independent social work license in the state of residence. Clinical licensure is preferred.
• Certified Case Manager (CCM) preferred.

Additional Qualifications:

• At least 5 years of clinical nursing or social work experience, with a strong focus on behavioral health.
• Demonstrated experience working with vulnerable and complex populations.
• Behavioral health experience is required. Relevant backgrounds may include:
  • Inpatient or outpatient behavioral health
  • Community mental health
  • Mental health clinics
  • Behavioral health case management
  • Substance use disorder treatment
  • Behavioral health nursing
  • Clinical social work
