Author Health, LLC is seeking a Care Manager (RN) to coordinate care for adults with behavioral health needs in a fully remote role across the United States. You will assess patients, remove barriers to care, develop personalized plans, and ensure safe transitions from facilities to home with a focus on reducing readmissions.

You will work with psychiatrists, nurse practitioners, social workers, and field staff, documenting progress and adapting plans to patient goals.

Qualifikationen

• Proven ability to perform comprehensive behavioral health and medical assessments.
• Strong interpersonal and communication skills to collaborate with patients and families.
• Ability to work independently within a multidisciplinary team.
• Proficiency with electronic health records (EHR) and health IT systems.
• Experience coordinating care transitions and patient-centered care plans.

Aufgaben

• Perform comprehensive assessments of patients' behavioral health and medical needs.
• Remove barriers to engaging in health care services to ensure medication and treatment adherence.
• Create personalized care plans in collaboration with patients, caregivers, and providers.
• Oversee execution of care plans to meet patient goals and objectives.
• Coordinate medication reconciliation and ensure accurate documentation during care transitions.
• Educate patients on health conditions, options, and self-management strategies.
