Job summary

Vynca Inc. is seeking a Behavioral Health Case Manager (internal title: Clinical Lead Care Manager, CLCM) to coordinate care for clients with complex needs.

Under the ECM Clinical Manager, you’ll work with doctors, social services, and family to align care plans, benefits and timely access to services, promoting wellness and independence. This is a remote, full-time role open to applicants in AZ, CA, CO, IL, NV, OR, TX, UT, WA.

Qualifications

• Active, unrestricted CA license (LCSW/LMFT/LPCC) required.
• 1+ year of care management, care navigator, or community health worker experience with vulnerable populations.
• Excellent oral and written communication skills.
• Bilingual (English/Spanish) highly preferred.

Responsibilities

• Assess member needs across physical health, mental health, SUD, social supports and housing.
• Develop and monitor client care plans and SMART goals.
• Provide care where member resides (office, telehealth or field-based).
• Coordinate with providers and social services to ensure timely, appropriate care.
• Document progress and report monthly as required.
