Job summary

Akron Children’s Hospital is seeking a Care Coordinator for behavioral health services. The role involves assessing needs, coordinating care with families and a treatment team, and monitoring progress for patients in ACH Pediatric Clinics.

Services are delivered in person and remotely to support cross-coverage across ACHPS. The candidate will identify acute risk of suicide, arrange crisis referrals, and support navigation across care levels.

Qualifications

• Experience working with multidisciplinary teams.
• Proficiency in Electronic Health Record documentation is preferred.
• Experience in healthcare is preferred.
• Excellent communication skills, both verbal and written, coupled with strong listening skills.
• Strong organizational skills with the ability to multi-task and problem solve.
• Proficiency in MS Office (Outlook, Excel, Word, PowerPoint) is required.

Responsibilities

• Identify and triage patients at acute risk of suicide and arrange crisis services.
• Participate in case conferences to co-develop treatment plans.
• Monitor symptoms and track treatment progress; coordinate appropriate care.
• Navigate between levels of care within Behavioral Health.
• Work with families to arrange appointments and address barriers to engagement.
• Provide education about behavioral health conditions and support treatment.
