Job summary

Cincinnati Children’s is seeking a Behavioral Health Care Coordination Specialist to engage youth and families in care coordination, convene Child and Family Team meetings, and develop CFCCPs. The role emphasizes collaboration with families, teams, and providers to implement safety plans and coordinate services.

The candidate will work primarily remotely with community-based fieldwork, including in‑person meetings, documentation in EHRs, and coordination with Aetna for quality review.

Qualifications

• Bachelor’s degree plus 2 years’ experience, or Master’s degree with 1 year experience in related areas (family systems; community resources; case management; child development).
• Must have reliable transportation.
• Must reside in Scioto/Lawrence County area.
• Wraparound experience; Meeting facilitation experience; Community-based experience.

Responsibilities

• Engage youth and family in forming the CFT and IS planning.
• Facilitate and convene CFT meetings; ensure meetings are convenient for the family.
• Serve as the facilitator of the CFCCP with team collaboration.
• Act as the point person for CFCCP implementation and monitoring.
