Job summary

Cincinnati Children’s is seeking a remote Care Coordinator to develop and implement individualized plans for youth and families within the PS Network—OhioRise. You will coordinate referrals, monitor care, and ensure services respect family goals and rights.

The role involves collaboration with clinicians, schools, and community partners, with a focus on trauma-informed and culturally competent care delivery in a remote setting.

Qualifications

• Bachelor’s degree and 2+ years’ experience or Master’s degree with 1 year experience in family systems, community resources, case management, child development or related fields.

Responsibilities

• Develop and implement an individual crisis and safety plan with youth and family.
• Facilitate referrals and linkages; monitor plan implementation.
• Manage 1915(c) Waiver funds ensuring prior authorization and ODM approval.
• Coordinate care with providers, schools, and community partners; ensure culturally competent services.
• Lead or participate in CFT meetings, complete CANS assessments, and engage families as partners in care.
