Job summary

Community Support Services in Akron, OH is recruiting a Nurse Care Coordinator to join the Regional Case Management Team. The role involves nursing assessments, care coordination with health providers, and ongoing evaluation of services to ensure appropriate clinical care.

The ideal candidate will have an Ohio RN license, experience with mental illness in community/clinic settings, a valid driver's license, and strong organizational and communication skills.

Qualifications

• Graduate of accredited nursing program with Ohio RN license.
• Experience delivering nursing care to individuals with mental illness in community or clinic settings.
• Car and valid Ohio driver's license; insurability through the agency.
• Knowledge of Ohio nursing rules, scope of practice, and related regulations.
• Excellent organizational, communication, and time-management skills.
• Ability to work in a team and maintain confidentiality (HIPAA).
• Proficiency in Microsoft Office.

Responsibilities

• Perform nursing assessments to identify risks (BP, BMI, etc.).
• Serve as liaison between public/private healthcare facilities and service agencies.
• Participate in interdisciplinary treatment team meetings and utilize community resources.
• Incorporate disease-specific goals into treatment plans and evaluate progress.
• Conduct home visits to assess clients in their environments.
• Provide crisis intervention and education to foster self-management.
• Accompany clients to complex medical appointments.
• Assist Nursing Supervisor in maintaining clinic staffing and operations.
• Work full-time during normal business hours.
