Job summary

Cincinnati Children’s is seeking a care coordination professional to support the PS Network - OhioRise team. The role focuses on care delivery, plan-of-care development, and coordination with youth, families, and community resources to ensure rights and goals are respected.

You will work remotely with day shifts, 40 hours per week, and a mix of ICC/MCC caseloads, applying trauma-informed and wraparound approaches to improve outcomes.

Qualifications

• Bachelor’s degree and 2+ years’ experience in family systems, community resources, case management, or related fields.
• May require clinical license, certification or training.
• Experience with Child and Family services or therapy preferred.

Responsibilities

• Care Delivery: develop and implement an individual crisis and safety plan with youth and family.
• Plan of care: implement and document patient response to individualized care plans.
• Therapeutic Relationships/Communication: coach and develop new staff and engage family in CFT meetings.
• Documentation and Data Management: complete CFCCP documentation and bill for care coordination.
