Job summary

Children's Healthcare of Atlanta is seeking a registered nurse to provide clinically based care coordination for patients with behavioral and mental health needs. The role includes assessment, discharge planning, and coordination of community services for patients transitioning from our facilities.

The position features 4 × 10-hour days (7:00 AM–5:30 PM) with an option to work remotely one day per week, and some weekend work as well as travel to other hospital locations may be required.

Qualifications

• RN with BSN or an RN with an Associate's degree who commits to earning a BSN within 2.5 years
• 3 years of broad clinical experience, preferably in pediatric or behavioral/mental health care
• Current Georgia license or multi-state license through eNLC is required or within state of Georgia

Responsibilities

• Serve as a system clinical resource for Care Coordination Assistants during scheduled shift
• Develop and implement robust transition care plans with the clinical team
• Coordinate discharge planning and community-based services for discharged patients
• Maintain effective communication with multidisciplinary team to drive care coordination
• Facilitate care conferences, rounds, and EMS transport escalations
• Complete medication prior authorizations and arrange prescriptions when needed
• Refer cases to risk management or quality departments as appropriate
