Centene is seeking a Behavioral Health Case Manager to assess and coordinate care for adults with complex mental health needs. The role emphasizes home visits, member education, and resource coordination in Navarro County, TX, with mileage reimbursement and a standard M-F schedule.

Requires a Master\'s degree in Mental Health or Social Work or Nursing, plus 2–4 years of related experience. Licensure as LCSW/LMFT/LMHC/LPC or RN with BH experience is required.

Formación

• Requires a Master\'s degree in Mental Health or Social Work or graduate from an accredited school of nursing with 2–4 years of related experience.
• 3+ years of direct clinical behavioral health experience in case management, care coordination, treatment planning, crisis intervention, and/or discharge planning.
• Experience as a community or field-based case manager or care coordinator is a plus.
• Strong clinical documentation and problem-solving skills; ability to adapt to business changes.

Responsabilidades

• Evaluate high-risk members with mental/behavioral health needs and recommend care plans.
• Act as liaison between member/family, physician, and facilities/agencies.
• Support members with mental/behavioral health needs and coordinate visits (monthly or more).
• Provide education to members and families on preventive care, treatments, and benefits.
