Job summary

Centene Corporation seeks a Behavioral Health Case Manager to develop and coordinate child/adolescent-focused BH care management. You will assess complex needs, coordinate with families and providers, and ensure access to community resources and services.

The role includes field/home visits, education on care plans, and adherence to clinical guidelines. A BH license or RN with BH experience is required; travel is involved across assigned counties.

Qualifications

• Requires a Master’s degree in Mental Health or Social Work or nursing graduate with BH experience.
• 2–4 years of related behavioral health/mental health experience.
• Preferred 5+ years of Behavioral Health Case Management experience with child/adolescent members.

Responsibilities

• Evaluates the needs of the most complex and high-risk members with mental/behavioral health needs and recommends a plan of care.
• Acts as liaison between member/family, physician, and facilities/agencies.
• Supports members with mental/behavioral health needs including depression, bipolar, schizophrenia, PTSD, substance use disorders.
• Performs frequent home visits to assess needs and collaborate with resources as required.
• Educates members and families on preventive care, treatment options, and benefits; supports BH-focused activities.
• Coordinates community resources and service authorizations for services based on assessments and plans.
• Ensures referrals are accessible and coordinates providers and resources.
• Maintains documentation and ensures compliance with guidelines.
• Provides feedback to leadership on quality of care improvements.
• Performs other duties and complies with policies.
