Qualifications

• Requires Master’s in Mental Health or Social Work.
• Licensed Behavioral Health Professional or RN with BH experience per state requirements.
• 2–4 years of related behavioral health/mental health experience.

Responsibilities

• Evaluate needs of complex, high-risk members with mental/behavioral health needs.
• Coordinate care plans and educate members and families on services.
• Conduct home visits and coordinate with community resources.
• Maintain documentation in electronic care management system and ensure compliance.
