Job summary

Qualifications

• Requires a Master's degree in Mental Health or Social Work or Nursing and 2–4 years of related experience.
• Preferred: 3+ years direct clinical behavioral health experience in case management or care coordination.

Responsibilities

• Evaluate needs of the most complex and high-risk members with mental/behavioral health needs and recommend a plan of care.
• Act as liaison between member/family, physician, and facilities.
• Perform frequent home or site visits to assess needs and coordinate resources.
• Provide and facilitate education to members and families on care services and benefits.
