Centene Corporation is seeking a Care Manager to develop and coordinate complex care plans for members with behavioral health needs. The role emphasizes education for members and families, collaboration with providers, and outcome-focused care management.

The position allows up to 75% local travel and remote work, with a Monday–Friday schedule and a sign-on bonus. Licensed BH professional or RN with BH experience is preferred. Compensation ranges reflect experience and location.

Qualifications

• Requires a Master’s degree in Behavioral Health or Social Work.
• Or a degree from an accredited school of nursing with BH experience.

Responsabilités

• Evaluates member needs via phone or in-home visits and facilitates care plans.
• May conduct telephonic, digital, or home visits outreach to assess needs.
• Develops ongoing care plans for high-acuity members and identifies providers and community resources.
• Coordinates between members, families, and care teams to ensure access to services.
• Monitors plans and outcomes and adapts care plans based on changes.
• Facilitates care coordination to ensure timely access to required care.
• Collects and maintains member information to ensure regulatory compliance.
• Educates members and families on procedures, options, and benefits.
• Provides leadership feedback on care delivery improvements.
• Performs other duties as assigned.
• Complies with all policies and standards.
